Adult Day Health
The Program
Our Adult Day Health Center offers an array of supportive services designed to prevent
or postpone the appropriate institutionalization of frail, homebound elderly or
functionally limited adults.
Our services are also available to individuals recently discharged from a hospital or
nursing home who can benefit from Adult Day Health programs to aid in the transition to
independent living.
Our goal is to keep the elderly in their "OWN" home by providing psycho-social
and health maintenance in a community setting.
